The Northern Arizona Healthcare Foundation is seeking applications for its Northern Arizona Community Health Grant program to address health priorities in northern Arizona: improving behavioral health outcomes, mitigating the impacts of chronic conditions, and increasing access to quality care.

Together, the Northern Arizona Healthcare Foundation and The NARBHA Institute commissioned the Center for Health Equity Research at Northern Arizona University to conduct a comprehensive assessment of health needs in their region. Through this process, they learned about the health issues their communities face, the causes of those issues, and the availability of resources to address them.
At the Foundation, they raise funds to affect change so these needs can be addressed. The aim is to increase the quantity and quality of health programs, services, and other resources to help members of the communities we serve overcome challenges associated with chronic illness, behavioral health, and access to quality healthcare.

To be eligible to apply for this grant program, organization must belong to one of the following categories: 501(c)(3) nonprofit organization, schools, faith-based organizations, tribal entities, and occasionally to government agencies.
Northern Arizona Community Health grants are awarded for innovative and collaborative health-focused projects of nonprofit organizations.
Nonprofit organizations seeking grant funding must serve some or all of the following counties: Apache, Coconino, Mohave, Navajo, and Yavapai.
Eligible organizations may submit an application to the Northern Arizona Community Health Grant cycle and the Arizona Community Foundation healthcare grant cycles of the Flagstaff, Yavapai, and Sedona.
Each year the Foundation confirms its community health needs priority which will be the focus of awards made through the year’s Northern Arizona Community Health Grant cycle. The Foundation Board of Directors may identify additional areas of interest.
The Foundation’s 2021 funding priority will be grant projects focused on improving community health through innovative, impactful and collaborative grant projects.
The 2021 Northern Arizona Community Health Grant cycle will prioritize projects requesting $50,000 or more.
Grant applications are submitted through the Northern Arizona Community Health Grant cycle administered by the Arizona Community Foundation (ACF).
